Modern implantology has overcome past times where delayed and early implant loading or extensive bone reconstruction were mandatory in critical scenarios, extending the overall treatment time. The development of new products and the application of new technologies based on new scientific evidences, have led us to elucidated new standards of success, reduction of complications, facilitate clinical procedures, and improve the well-being of our patients.
Innovative implant systems have been born to push the envelope of current implantology, going forward to provide an easier clinical solution and providing new properties that make these implants the election in favorable and unfavorable scenarios.

In recent decades, immediate placement has evolved considerably, becoming a highly predictable treatment for the replacement of lost teeth. With the aim of reducing surgical times and costs for our patients, immediate placement emerges as an ideal option. However, a particular challenge arises in the case of molars, where the correct three-dimensional position of the implant may be compromised. This presentation will address the various questions that arise when considering immediate placement in the molar area, providing guidelines for making the best clinical decision. In addition, the integration of digital dentistry will be discussed and how it has contributed to making our treatments more predictable and efficient.

Dental implants have revolutionized solutions for both completely edentulous and partially edentulous patients, boasting a high success rate. Since their inception, dental implants and associated accessories have undergone dramatic transformations, allowing for immediate placement and, in many cases, immediate loading in a simplified manner. However, challenges persist, particularly in cases involving atrophic ridges, necessitating guided bone regeneration. While some cases of bone augmentation are predictable, others pose control challenges, demanding intricate techniques and a high level of expertise, alongside the associated discomfort and financial costs. These challenges are particularly pronounced in medically compromised or elderly patients, potentially leading to decreased case acceptance.Recent modifications in implant design have introduced simplified alternatives for severely atrophic ridges. These advancements have ushered in a revolutionary solution for enhancing the dental health of patients, leading to increased levels of case acceptance and patient satisfaction.

Digital workflow in implantology has allowed the development of less invasive treatments, with greater precision and optimizing clinical outcomes. R2GATE is a surgical and prosthetic planning tool for the correct positioning of dental implants that minimize risks and facilitates the prosthetic stage.This lecture considers clinical experiences of incorporating R2GATE for all implant cases, clinical protocols for obtaining records, a simplified protocol for the matching process and the clinical planning process. The advantages of the combination of R2GATE and Guided Surgery will be shown for different clinical situations from the simplest to the most complex, such as delayed implants, immediate implants in the esthetic zone, multiple implants, and full arch cases. A guided surgery protocol with irrigation access for bone drilling will be presented as a tip for improving surgical performance for guided implants.
