This clinically oriented presentation explores the application of BD Cuff® and ARi® implant systems and immediate molar implant placement and management of challenging posterior extraction sites. Through a combination of clinical cases, treatment-planning strategies, surgical protocols, and restorative considerations, the course will demonstrate how implant selection and implant positioning can be adapted to the available bone, soft-tissue dimensions, extraction defect morphology, and prosthetic requirements. Particular attention will be given to the clinical rationale behind the BD Cuff® concept for compromised posterior sites, including thin ridges, bone defects, and situations in which conventional implant placement may require additional augmentation. The presentation will also examine the ARi® implant concept and its approach to obtaining primary stability through engagement of available basal bone in challenging ridges. A major component of the course will focus on immediate molar implant therapy, including socket evaluation, three-dimensional implant positioning, management of interradicular and buccal defects, selection of implant dimensions, primary stability, soft-tissue management, provisionalization, and criteria for immediate versus delayed loading. Real-world clinical cases will be used to illustrate what works, what can go wrong, and how treatment protocols can be modified according to individual clinical circumstances. The goal is to provide clinicians with practical decision-making tools that can be incorporated into everyday implant practice.

Course Objective

• Identify appropriate clinical indications for BD Cuff® and ARi® implants based on bone availability, ridge anatomy, soft-tissue dimensions, and prosthetic requirements.
• Understand the design principles and clinical rationale behind the BD Cuff® and ARi® implant concepts and how these designs can be applied to challenging clinical situations.
• Evaluate immediate molar implant candidates through clinical and CBCT assessment, including socket morphology, interradicular anatomy, buccal bone integrity, soft-tissue phenotype, and proximity to vital structures.
• Develop a practical treatment-planning protocol for immediate molar implant placement, from atraumatic extraction through implant positioning, grafting when indicated, and provisional or definitive restoration.
• Apply surgical protocols for compromised posterior sites, including thin ridges, extraction defects, dehiscence, and limited alveolar bone.
• Recognize when a BD Cuff® approach may simplify treatment and when conventional augmentation, alternative implant designs, or staged treatment may provide a more predictable solution.
• Understand the clinical application of the ARi® concept in anatomically challenging and atrophic ridges, including strategies for achieving primary stability through available basal bone.
